Support at Home Sydney | Approved Aged Care Provider | Muslim Care

Support at Home – the new home aged care program

On 1 November 2025, the Australian Government’s Support at Home program replaced Home Care Packages (HCP). Muslim Care is an approved provider delivering Support at Home services across Sydney, helping older Australians stay safe, healthy and independent in their own homes.

If you already had a Home Care Package, you were moved automatically to Support at Home and are protected by the government’s “No Worse Off” principle — your funding and contributions will not be reduced by the change.

What is Support at Home?

Support at Home is the Commonwealth program that funds in-home aged care. Instead of four package levels, there are now eight classification levels, so funding better matches your assessed needs. Budgets are managed quarterly, and services are grouped into clinical supports, independence supports and everyday living supports.

Services we deliver under Support at Home

  • Personal care – showering, dressing and grooming
  • Domestic assistance – cleaning, laundry and meal preparation
  • Nursing care at home
  • Transport to appointments and shopping
  • Social support and community outings
  • Respite care for carers
  • Allied health and wellbeing services
  • Home maintenance, aids and equipment

Our team delivers culturally appropriate care with multilingual support workers, serving families of all backgrounds across Sydney.

How to get started

  1. Apply for an assessment through My Aged Care (myagedcare.gov.au or 1800 200 422).
  2. Receive your classification – an assessor will determine your support level and quarterly budget.
  3. Choose Muslim Care as your provider – call us on (02) 8011 0911 and we will build your care plan and start services.

Already have a Home Care Package or CHSP?

Existing HCP clients transitioned to Support at Home automatically. The Commonwealth Home Support Programme (CHSP) continues for entry-level support and is planned to join Support at Home from July 2027. We can help you understand exactly what the changes mean for you — see our Home Care Packages and CHSP pages, or visit our FAQ.
